Introduction to Session Notes
A session note is one of the most important documents you must use when assigned to a case. Session notes are used to bill your sessions held with a child and an important part of our State, Fiscal, and City audits. For that reason, session notes must be accurate, high quality, and submitted to the agency promptly. 
With the exception of OTAs, Bright Future currently gives our therapists TWO options for completing session notes (handwritten OR fully electronic).

Both options (handwritten OR fully electronic) have many similarities but there are also key differences, therefore, it is imperative that you understand the procedure for whichever option you choose. 
Before you do anything else, please review the session note samples here. As you can see, a session note is to include everything that happened during the session including the date of the session, the time of the session, who was present during the session, the activities that occurred within the session, and how the parents should reinforce learning activities. 
Handwritten Session Notes
Below is the procedure if you are choosing to do handwritten notes:
  1. Complete a session note. To know how to complete a handwritten session note, please follow the session note samples here.
  2. Enter your data entry claim into Providersoft. In order to know how to enter a data entry claim in Providersoft, please click here.
  3. Complete a session note LOG (IF  your case is telehealth). Click here for info on telehealth cases, a blank log, and a sample log
  4. Submit your session notes to us based on our Billing Submission Cycle shown herePlease note - you will need to send originals OR copies of your handwritten session notes dependent on IF you are a non-DOH or DOH provider. Please click here for further information.  
IMPORTANT: As a courtesy, pre-filled notes with the child's name, your name, & IFSP goals will be sent to you approximately 2 weeks from the date that you were first assigned the case. This SHOULD NOT prevent you from starting your case. Click here for a blank session note to use while you wait for your pre-filled note.

Electronic Session Notes
Below is the procedure if you are choosing to do fully electronic notes:
  1. Complete a session note via the Providersoft system only. Click here for the Providersoft training video.
  2. Complete a session note LOG (IF  your case is telehealth). Click here for info on telehealth cases, a blank log, and a sample log
IMPORTANT: We have not yet been able to enter the child's goals into Providersoft for you to select. This SHOULD NOT prevent you from starting your case. Simply type the goals manually on your electronic note OR copy and paste the goals from the IFSP into the electronic note OR click here for instructions on how to add the child's goals into Providersoft.
